^Annual tune-up for Whole House and Home Comfort plans; Select Service plans receive a 20% discount on an annual heating/air conditioning system tune-up. Plumbing/Electrical plan has a $50 deductible per item. Following two-month promotional period, standard monthly rate applies. Coverage begins 30 days from the submittal of this form and continues for an initial period of 12 months and will continue thereafter in perpetuity on a month-to-month basis. Each monthly period will be treated as a separate Contract period. Membership will be billed monthly and shall remain in effect until cancelled in the manner described within the following:  Terms and Conditions and Brochure.
